Important: This form must be completed by every guest 18 years or older. Guests under 18 must have a parent or legal guardian complete and sign the waiver on their behalf.

This is mandatory for all guests — not optional. Guests who arrive without a signed waiver will not be permitted on the property.

If you are not comfortable signing the waiver, contact us right away and we will cancel your reservation with a full refund.
